NEW PLAYERS - 2012 NEW PLAYERS MUST REGISTER IN PERSON (PARENT ONLY IS FINE) ON ONE OF OUR REGISTRATION DAYS MENTIONED ABOVE
1. Download and complete NEW PLAYER FORM
2. You MUST read and agree to the Terms and Conditions for membership and tick off on the rego form.
3. You MUST supply a photocopy of birth certificate or passport as proof of age.
4. Registration fee - please see below. Payment by Cash or Cheque/Money Order only - NO CREDIT CARDS ACCEPTED

RETURNING PLAYERS in 2012 - Returning Players from 2011 (Boys U6/U7/U8/U9 and U10) should now have received their rego email.
All other age-groups will receive registration information by Monday 6th February. Due to a computer glitch we have been slightly delayed.

U5 to U7 --- $250
U8 to U9 --- $260
U10/U11 -- $275

Additional 1st sibling receives a $25 discount, Additional 2nd Sibling $60 discount, Additional 3rd Sibling free

Registration forms without payment will not be processed. We accept Cash, Chq/Money order
(NO CREDIT CARDS ACCEPTED).

Please Note:
Queens Park Football Academy has a NO REFUND policy. Once registration is complete, fees will only be refunded if a player cannot be placed in a team.
Queens Park Football Academy has a NO PAY NO PLAY policy. No player can join a team, train or play until fees have been paid in full.

We welcome all players born in 2007 or before. The regulation states that whatever age a player turns in 2012 (1st Jan to 31st Dec) becomes the age group for football:

QPFA provides the convenience of same time, same place matches every week for both boys and girls from U5 to U7 which allows the busy family to plan their w/e.

BOYS
Boys U5/U6 - Saturday 8.30am to 9.30am
Boys U7 - Saturday 9.30am to 10.30am

Indication of match times for
Boys U8 - Saturday 8.40am to 10.30am
Boys U9 - Saturday 9.40am to 11.30am
Boys U10* - Saturday 10.40am to 2.00pm
Boys U11* - Saturday 11.40am to 3.00pm
*U10/U11 timeslots will be worked out after team allocation, taking into account teams preferences and private school commitments.

Boys and Girls U8 to U11 will also have the fantastic opportunity during the season of playing at Hensley Athletic Ground on Astro Turf with Grand Stand and state of the art facitilites on a Wednesday.

LAST GAME AND GALADAY/TROPHY PRESENTATION in August - date to be confirmed

• Complete Club Uniform ($50) required for SSF players
  (Shirt, Shorts and Socks)

Additional items bought separately

• Club Shorts ($20) and Club Socks ($10)

• Boots and Shinpads are mandatory and can be purchased from any retailer.
  (Boots must have moulded studs – no metal studs are allowed)

CLUB UNIFORM Girls G10 and G11 Champions League Uniforms for sale through QPFC only - DATES TBA
Champion League Uniform can be discussed as an alternative to the QPFA Uniform for this agegroup.

WORKING WITH CHILDREN CHECK for Coaches and Managers

Coaches and manager must download and complete the QPFA/ Working with Children Check
Please return these to Queens Park Football Academy, 32 Lawson St, Bondi Jct 2022 before the start of the season.

NEW and RETURNING Coaches and Managers also need to email a digital photo (head and shoulders only) to the Registrar on admin@qpfa.com.au
